Rhinoplasty for the Long Nose

My rhinoplasty patients from the Dallas - Fort Worth area frequently complain that their nose is too long or too big. To the rhinoplasty surgeon, length relates to the distance along the bridge down to the nasal tip. It implies a distance greater than the stomion to menton length and a nasolabial angle less than 90 degrees. While their are specific medical guidelines about whether a nose is technically "long," it is sufficient that you are bothered by your nose appearance and that you feel it does not appear to be in proportion with the rest of your face.

Correction of a long nose involves rotating the tip complex back onto the septum and midvault structures. If lining and cartilage is removed, care must be taken to avoid over-resection that could create a short nose deformity. Careful analysis and planning is critical to these decisions.

I frequently use open rhinoplasty techniques to enable direct suturing and repositioning of structures thereby minimizing the irreversible need for resecting tissue.
