Revision Rhinoplasty in Dallas

For most patients who have rhinoplasty in Dallas, the procedure is relatively straightforward. It might be that there's a bump on the nose that the patient doesn't like or that their nose 'hooks' at the end and they really don't like that. Dr. Byrd sees a relatively high number of patients who have already had one or more rhinoplasty surgeries elsewhere who are still unhappy with the look of their noses. More importantly some patients have had numerous surgeries and are left with breathing problems. There are also adults who had surgery as infants to address congenital cleft palate deficiencies. Dr. Byrd is one of the few surgeons in the United States who will evaluate and perform reconstructive surgery using cartilage grafts for patients who are left in these uncomfortable and unhappy situations.

Initial Evaluations

There is a very valid reason why Dr. Byrd is one of the few surgeons performing secondary and revision rhinoplasty in Dallas / Fort Worth. There are a high number of these cases where there is very little if anything that can be done to correct the residual problems from a prior surgery. Surgery is not an option for every patient and we make every effort to determine this early on, pre-screening patients before they commit to an in-office consultation.

As part of the pre-screening we ask patients to send in copies of their operative reports as well as pictures taken before and after each surgery. They'll also be asked to send a detailed list of what they expect to achieve through another nose surgery. After Dr. Byrd and the medical team have had an opportunity to study this information, a decision is made whether to schedule a consultation with Dr. Byrd or to explain to the patient why another surgery would be ineffective in achieving their goals. We're happy to evaluate each patient's unique circumstances and determine if there's a way for us to improve the patient's current situation.

Secondary Rhinoplasty Cases

For some patients who have had two or more rhinoplasty procedures, they are not only left with undesirable outcomes, but are often left with weakened, deformed, and/or malpositioned supporting cartilage. It is common for these patients to have internal scarring which can severely limit possible future corrections.

One of the biggest challenges in secondary rhinoplasty cases is finding appropriate replacement cartilage for the structures that have been weakened or removed in previous surgeries. Septal (nose) cartilage is always the first choice, but is rarely available in secondary rhinoplasty patients. The majority of the time any possible septal cartilage was removed during the first surgery. When septal cartilage isn't available, Conchal (ear) grafts are evaluated as an appropriate substitute. Conchal grafts are a good choice to use in small areas and where only light support is needed, or whenever "fill," shaping or camouflage is needed. However, in many cases so much structural support of the nose has been removed that Conchal grafts aren't strong enough to support the necessary corrective surgery. In those cases, or whenever strong structural support is necessary, cartilage from the ribs is harvested and used to support and shape the nose.
